Episode 118: Spill It " When Is the Right Time — and Who Gets to Decide?"

Written by Sandra Silverman | Jan 21, 2026 4:32:19 AM

One of the reasons I love Spill It is because it brings us back to what really matters: real people, real questions, and real life.

This week’s letter came from a woman asking about her 18-year-old niece, who admires her aunt’s nose and is thinking about getting her own redone. The question was simple — Is that too young? — but the answer isn’t.

I want to be clear: there’s no single “right” age for cosmetic procedures. What matters far more is why someone wants to do it, how informed they are, and whether the decision is coming from confidence or insecurity.

At 18, emotions are still forming. Identity is still evolving. And while some young people are incredibly self-aware, others are responding to pressure — from social media, comparison, or the belief that fixing one thing will fix everything.

That’s where guidance comes in.

Cosmetic decisions aren’t just physical. They’re emotional. They affect how someone sees themselves for years — sometimes for life. That’s why conversations, not just consultations, are so important.

In Spill It, Vita and I talk about slowing down. Asking questions. Making sure decisions are grounded in self-respect rather than urgency or outside influence.

There’s no judgment here. Only experience.

If someone chooses to move forward, it should be because they understand the process, the permanence, and the emotional weight — not because they’re trying to become someone else.

You don’t need to rush becoming yourself.

And when a question like this comes up, the most powerful thing we can offer — especially to younger women — is honesty, patience, and perspective.

That’s what Spill It is all about.
